Poinsettia plants will decorate the sanctuary this Christmas season. The cost of each plant is $10.00. If you would like to donate a poinsettia in memory of, or in honor of a loved one, please complete and return this form to the church office by Sunday, December 15 along with a check payable to Lakewood Presbyterian Church. If you want to pick up your poinsettia on December 24 after the 8pm Christmas Eve worship service, please indicate on the form.
